Documenting Discussions

Understanding how to document conversations about advance care planning is important. During this session, you’ll do just that, along with learning how to capture the information needed for new billing codes and documentation details. Participants will be able to identify challenges in tracking advance care planning documents and document effectively for reimbursement. Links to online resources will be provided.

Objectives
Following Part 3 of this learning activity, participants will be able to:

  • Discuss pitfalls of conflicting advance care planning documents.
  • Describe key elements of proper documentation for reimbursement.
