Change Package Strategy 6: Provide Exceptional Compassionate Care That Treats the Whole Person
A focus on the whole person requires that staff know the residents well and can anticipate their needs. It also requires an engaged and competent medical and care team that effectively manages residents’ changing health conditions and avoids healthcare-acquired conditions (HACs).
To provide exceptional, compassionate care that treats the whole person, Strategy 6 of the National Nursing Home Change Package provides four change concepts:
6.a. Implement Consistent Assignment
6.b. Choose and Engage Medical Leadership Wisely
6.c. Transition with Care
6.d. Strive to Prevent Problems and Treat When Necessary
Together, strategies and change concepts, along with action items are designed to lead to improvement.
For example, to help implement Change Concept 6.c.: Transition with Care, some of the these action items are provided:
- Create a structure and processes for communication to ensure key information is consistently transferred from staff to staff.
- Ensure that all changes in resident status have been communicated by having staff round together at the change of a shift.
- Collaborate with referring hospitals to identify needed information at the time of admission to the nursing home and transfer to the hospital.
