Care about Your Care
In 2011, RWJF launched Care About Your Care to focus attention on what people can do to provide and receive better health care. The conversation has centered around improving care transitions to reduce avoidable hospital readmissions, and how nurses, care coordinators, doctors, consumers, caregivers, patients and others can work together to achieve this.
According to The Robert Wood Johnson Foundation (RWJF):
- more than one-third of us don’t get the tests, referrals or follow-up care we need after a hospital stay
- one in five patients with Medicare is readmitted to the hospital within a month of discharge
- millions of preventable trips to the hospital occur
