Get Help Transforming Your Practice
Clinicians who want to be a part of the Transforming Clinical Practice Initiative (TCPI) often don’t know where to start. Now practices in Alabama, Indiana, Kentucky, Mississippi and Tennessee have a new resource developed by the atom Alliance to connect them with Practice Transformation Networks (PTNs) in their area.
TCPI is one part of a strategy advanced by the Affordable Care Act (ACA) to strengthen the quality of patient care and spend healthcare dollars more wisely. The TCPI is designed to support more than 140,000 U.S. clinician practices during the next four years in sharing, adapting and further developing their comprehensive quality improvement strategies. It aligns with the criteria for innovative models set forth in the ACA:
- Promoting broad payment and practice reform in primary care and specialty care,
- Promoting care coordination between providers of services and suppliers,
- Establishing community-based health teams to support chronic care management, and
- Promoting improved quality and reduced cost by developing a collaborative of institutions that support practice transformation.
The PTNs are peer-based learning networks designed to coach, mentor and assist clinicians in developing core competencies specific to practice transformation. This approach allows clinician practices to become actively engaged in the transformation. It also ensures collaboration among a broad community of practices that creates, promotes and sustains learning and improvement across the healthcare system.
This map also provides state and regional level PTN information for clinicians looking to begin their transformation journey. Click on a state to find PTNs in your area and then select a PTN name to link to contact and recruitment information about a specific PTN.
The atom Alliance helps the PTNs engage practices – especially those that are rural, small or reach medically underserved populations — in advancement of the TCPI goals. For more information, contact Jennifer Ride, phone (615) 574-7221.
